肝性胸水
Hepatic hydrothorax
[介紹]
指 排除了其他原因(心/肺/胸膜疾病)的 肝硬化者的 肋膜積水(通常>500ml)
5-15%的肝硬化患者有Hepatic hydrothorax
多伴隨 腹水/肝腦病變/AKI/死亡風險↑
腹水5-10L症狀可能仍輕微
胸水500ml就可以有嚴重症狀
[機轉](尚不完全了解)
可能是腹水藉由一些微小橫膈膜肌肉的缺損(small diaphragmatic defects)進入肋膜
這些缺損通常
1.<1公分(may be microscopic)
2.位於膈膜腱的部份(tendinous portion of the diaphragm)
3.更常發現於右半膈膜(佔73-85%)(左邊比較厚/多肌肉構成)
因為胸內負壓/加上腹腔壓力上升,會促使液體從腹腔進入胸膜腔(因此有些病人沒有明顯腹水)
**Studies: 99mTc-human albumin / 99mTc-sulphur colloid 腹->胸膜 單向通過
當積水量超過肋膜可吸收的量時，Hepatic hydrothorax就會變的明顯
似腹水可能會發展成SBP自發性細菌性腹膜炎
Hepatic hydrothorax患者有機會發展成spontaneous bacterial empyema(SBEM自發性細菌性膿胸)
機轉:SBP或菌血症擴散來的
[診斷]
-判斷有胸水(肋膜穿刺/影像學)
-排除其他可能原因
-注意有沒有感染
-出現在右邊的機率>左邊
Thoracentesis肋膜穿刺
胸水fluid testing:
●Cell count and differential
●Gram stain
●Culture->建議bedside直接放入culture bottle中(對SBEM的sensitivity會比較好)
●Protein, albumin, LDH, bilirubin
●Pleural fluid pH
+ 抽血:serum albumin, LDH, bilirubin
其他鑑別診斷可能會用到的:
●Triglyceride level (chylothorax)
●Polymerase chain reaction for Mycobacterium (tuberculosis)
●Amylase concentration (pancreatitis)
●Cytology (malignancy)
若是肋膜積液是原自門脈高壓，那應該會與腹水相似
-transudative
-low protein concentration (<2.5 g/dL)
-serum-to-pleural fluid albumin gradient >1.1 g/dL
但仍會有與腹水不同之處:因為腹膜腔與肋膜腔的吸收液體的機轉不同
-total protein and albumin可能會略高於腹水
Uncomplicated hepatic hydrothorax: PMN <250 cells/mm3
SBEM citeria
●Pleural fluid culture(+) and PMN >250 cells/mm3
●Pleural fluid culture(-) and PMN >500 cells/mm3
●No evidence of pneumonia on a chest imaging study

-CT:排除縱隔/肺/胸膜病變
-Abdominal sono(w/ doppler):看肝臟腫塊/門靜脈&肝靜脈通暢性/腹水
-ECG:排除心因性肋膜積水
以上診斷不明確時，
可在腹膜內注射99mTc-sulphur colloid / 99mTc-human serum albumin
適合用在剛把胸水抽完，等下一批胸水累積時注射

-Malignant effusions

hepatic hydrothorax必有cirrhosis
通常有腹水/在右邊/為transudative effusion
